Shaft voltage measured at the coupling exceeds the level the motor maker allows — Insulate at the non-drive end and earth the drive end. Insulating both ends leaves the current no path to earth except through the gearbox or the driven pump. Housing seat is a slack fit and the outer ring can creep — An OD-coated bearing is the wrong answer to a loose seat. The coating wears through at the creep point, the insulation is lost, and fluting starts in the same running hours as before. Machine is washed down or works in a humid coastal plant — Confirm the coating is the sealed execution. Moisture soaked into an unsealed ceramic layer drops the insulation resistance to near nothing while looking perfectly intact. Test the insulation after fitting, before the coupling goes on. A megger reading taken with the shaft already earthed through the driven machine tells you nothing, and that is the reading most sites end up recording.
