Bearing housing is running hot with no obvious mechanical fault — Circulation with a cooler sized on heat load rather than on flow. Grease or a static oil bath simply cannot carry that heat out of the housing. Process area generates fine scale or fibre that gets into the oil — Duplex filtration that can be changed on line, plus a magnetic element. Shutting a mill to change a single filter is how filters end up never being changed. Return lines have to run a long way to the tank — Generous return pipe bore with continuous fall, and check it on the drawing. Restricted returns flood the housing, and the seal then leaks oil onto the floor.
